Contract Nurse Practitioner:

Flat rate fee: $45 per hour

(that's LESS than your plumber!)
Strictly "work for hire" basis -- you only
pay for the hours actually worked
seeing your patients, not for
transportation, health insurance, or
taxes -- I take care of that myself.
I'll prepare a time sheet on Friday of the
week worked - payment is net 14 days
(1-1/2% on accounts over 30 days).

Absolutely no other fees, charges, or bills.

Because we will be in collaborative practice,
your staff bills the patient's payor at your usual
rate -- generally $78 for a mid-level encounter
that takes about 15-20 minutes. At that rate, I
might care for four mid-level encounters per
hour (say, four nursing home visits) -- a total of
$312 billable. After deducting my hourly rate,
you are still ahead $267.

Even if I "only" see one mid-level patient in an
hour, say, as a house-call visit -- you are still
ahead $33 after paying me. PLUS, you have
developed incredible patient good will and
satisfaction when your Contract NP makes a
home visit!

SOAP format notes, or other format, provided
back to you on the schedule you set.

Since the State of Missouri requires a calendar month of contact for formalized Collaboration to
exist, here's what I will offer after we've had a chance to talk this over:

meet in your clinic and sign the Collaboration Agreement
work that day at your side, learn your approach and system at no charge to you
return near the end of the calendar month and work at your side again, for a minimum
of four hours at no charge to you
NO COST -- I'll be there and you won't owe me a thing
NO RISK -- If you don't like the way I work, or anything else, then we'll end the
Collaborative Agreement right there.

And if you are satisfied with my performance, we can begin to schedule your overflow, time-
consuming, nursing home, or house-call patients right away.

I can see patients OUTSIDE the clinic, and IN the clinic -- go ahead, have your staff
schedule a full day of well-woman exams or sports physicals each month. I can care
for these in one of your exam rooms while you handle higher level patients.
FLEXIBILITY is the hallmark of Contract Nurse Practitioners -- I can be available on
short notice if you need to be out of the office for an emergency, or if there is a sudden
influx of patients.
During the busy flu and URI season, 24-48 hours notice for patient scheduling would
be appreciated -- we'll both be busy, but I do want to accommodate my regular
collaborators!
My GUARANTEE: If at any time you are dissatisfied with my services, we can end the
Collaboration Agreement on the spot. No obligation, no recriminations. If you're not
happy, I'm not happy -- I'll either make it better, or move on!
